A penile fracture is a term often heard less frequently, but it is actually an emergency requiring early intervention. It occurs when there is a rupture of the tunica albuginea, a thick and resilient membrane covering the corpora cavernosa within the penis. This unusual urological condition, while rare, has an incidence rate reported at approximately 1.2 to 10 cases per 100,000 men. Patients experiencing a penile fracture typically describe severe pain, swelling, bruising and a clicking sound, and are unsure about what went wrong and whom to approach.
Penile fractures most often result from a traumatic event during sexual intercourse when the erect penis inadvertently slips out and forcefully strikes against the perineum or pubic bone causing a forcible bend on the penis. This mishap can occur during various sexual positions. During erection, the thickness of the tunica albuginea diminishes, making it more susceptible to rupture when there is an abnormal bend. The tunica albuginea can withstand substantial pressure—approximately 1500 mmHg—before giving way. Other causes of penile fracture include forcible bending of an erect penis, vigorous masturbation, and, rarely, rolling over an erect penis.
